The Together 2 Goal® campaign is excited to invite you to apply for the new Innovator Track. The Innovator Track, our most advanced data-reporting track, builds on Core Track measures and will consist of two cohorts: the Cardiovascular Disease (CVD) Cohort and the Eye Care Cohort.
In each cohort, participants will collaborate with clinical advisors and AMGA staff over 12 months to develop a framework and metrics to improve either CVD management or eye care for their patients with Type 2 diabetes. We will select 10 organizations for each cohort via a competitive application process and will offer a $7,500 stipend to each selected organization.
Applicants must be Together 2 Goal® participants and must commit to reporting data on the Core Track. Organizations are welcome to apply for one or both Innovator Track cohorts. However, in selecting, the committee will pay close attention to an organization’s ability to manage two quality improvement projects simultaneously.

To apply, submit one electronic copy of the application to InnovatorTrack@amga.org. CVD Cohort Applications are due by 5:00 p.m. PST on February 28, 2018 and Eye Care Cohort Applications are due by 5:00 p.m. PST on March 16, 2018.
