Last month, Together 2 Goal® and our advisors selected 13 campaign participants to take part in the Cardiovascular Disease (CVD) Cohort of the Innovator Track. These groups will work together to identify innovative ways to identify and manage CVD risk for their patients with Type 2 diabetes.

The selected groups are: 

  • Geisinger
  • Hattiesburg Clinic, P.A.
  • Kelsey-Seybold Clinic
  • Mercy Clinic- East Communities
  • Premier Medical Associates, P.C.
  • PriMed Physicians 
  • ProHealth Physicians, Inc. 
  • Sharp Rees-Stealy Medical Group, Inc.
  • Southwest Medical Associates, Inc. 
  • Summit Medical Group, P.A.
  • SwedishAmerican Health System
  • Utica Park Clinic
  • Watson Clinic, LLP

The initiative, which is sponsored by Boehringer Ingelheim and Eli Lilly and Company, will kick off with an in-person meeting in Minneapolis in May and will continue over the course of 12 months as participants implement their projects and share their data.

In addition, advisors are currently reviewing applications for the Eye Care Cohort—the second cohort of the Innovator Track—and will make their selection of participants later this month.
