We are excited to announce that 10 groups were selected to participate in the Eye Care Cohort of the Innovator Track—the second of two Innovator Track Cohorts. These participants will develop and measure population-based interventions for managing eye care for people with diabetes.
The selected groups are:
- Ballad Health
- Baton Rouge Clinic
- Coastal Carolina Health Care, PA
- Geisinger
- Henry Ford Medical Group
- Kelsey-Seybold Clinic
- Prevea Health
- Utica Park Clinic
- Valley Medical Group
- Watson Clinic, LLP
Over the course of 12 months, participants will focus on improving collaboration with specialists and bridging data gaps—two significant challenges to managing eye health among this patient population. The initiative will kick off with an in-person meeting in Nashville in July.
