December 2018 Edition

This has been an eventful year for the Together 2 Goal^® campaign and we want to extend a huge “Thank You!” to all of you who have worked hard to improve care for people with Type 2 diabetes. In 2018:

2 Innovator Track Cohorts kicked off, mobilizing 22 participating medical groups and health systems to tackle cardiovascular disease and diabetic retinopathy
7 campaign participants were recognized as T2G award winners for achievement and improvement in diabetes measures
7 AMGA members spread lessons learned and best practices through T2G presentations at national conferences
11 new organizations enrolled in the campaign, resulting in 2,640 FTE physicians joining this national effort
23 partners, experts, and T2G participants presented on 17 T2G and Innovator Track webinars
28 diabetes teams hosted a T2G Talk & Taste for this year’s National Day of Action, engaging over 200 providers in conversations on diabetes improvement

Together, our efforts have resulted in improved care for more than 750,000 people with Type 2 diabetes! We are excited to keep up the momentum in 2019 and look forward to continuing efforts to reach our shared goal of 1 million lives improved!

Campaign Spotlight

We’re excited to welcome Mallory Temple as the Coordinator for the National Health Campaigns Team! Mallory double-majored in Economics and Public Health at Oregon State University and spent the last year working at The Coordinating Center, helping individuals with disabilities and complex healthcare needs. Mallory will be supporting the T2G webinar series, Innovator Track, and more, so join us in welcoming her to the T2G team!

Resource of the Month

The American Diabetes Association (ADA) Standards of Care are now available through their new phone application (app). The app includes the ADA 2018 standards for diagnosis and treatment of diabetes, nutrition principles and recommendations, and valuable guidelines on managing diabetes and its complications in various settings. This new app offers quick access to content and includes many interactive tools. Look for updates once their 2019 standards are released.
