The Together 2 Goal® campaign is thrilled to announce our Plank Mentors! Plank Mentors are individuals who have excelled at implementing one of the campaign planks within their organization. They are available to you to answer questions, offer expertise, and share lessons learned on a given campaign plank in a one-on-one setting. Our 11 Plank Mentors include:
- Build an Accountable Diabetes Team - Rae Ann Williams, M.D., FACP, of HealthPartners Care Group
- Integrate Emotional and Behavioral Support - Mark Greenwood, M.D., of Intermountain Medical Group
- Refer to Diabetes Self-Management Education and Support Programs - Valerie Spier, M.P.H., RD, CDE, of Sutter Health

- Assess and Address Risk of Cardiovascular Disease - Liana Spano-Brennan, D.O., FACC, of Summit Medical Group, P.A.
- Contact Patients Not at Goal and with Therapy Change within 30 days - Brian Jameson, D.O., of Geisinger
- Use a Patient Registry - Brian Shablin, M.D., M.S. of Lehigh Valley Physician Group
- Embed Point-of-Care Tools - Scott Hines, M.D., of Crystal Run HealthCare
- Publish Transparent Internal Reports - Stephen Combs, M.D., CPE, FACFE, FAAP, of Ballad Health

We are pleased to share that our very own John Cuddeback, M.D., Ph.D., chief medical informatics officer at AMGA, was featured in On Call, a podcast that highlights insights and innovations that drive change in the healthcare industry. In this episode, Dr. Cuddeback describes the creation of a predictive data model for pre-diabetes management and how similar models can be used in electronic health records at the point of care.

Check out this series of live, interactive web conferences led by experts in CKD sponsored by Janssen Pharmaceuticals. In addition to discussing prevalence, risk factors, and screening for CKD in patients with diabetes, these webinars will also consider patient burden and Medicare spending for patients with diabetes and CKD.
