December 2019 Edition

As 2019 comes to a close, we are thrilled to announce that together, we have improved care for 1,082,000 people with Type 2 diabetes! This means that we have officially reached (and exceeded!) our campaign goal of 1 million lives improved. We want to extend a huge “thank you!” to all of you who have worked hard over the past three years to accomplish this incredible achievement. While there is still plenty of work between now and March 2021 to hardwire care processes and infrastructure changes to make these improvements sustainable, let’s reflect on our successes over the past year. In 2019:

  • 4 T2G award winners were recognized for best performance by data track and size
  • 4 new partners and supporting organizations joined the campaign
    • Endocrine Society
    • Know Diabetes by HeartTM
    • National Kidney Foundation
    • National Minority Cardiovascular Alliance
  • 10 AMGA members and partner organizations contributed news tools and resources to the T2G Toolkit Supplement
  • 10 groups successfully completed participation in the Innovator Track Eye Care Cohort
    • 8,600 additional diabetes patients with documented screening for diabetic retinopathy
  • 11 Plank Mentors recorded Q&A videos and offered peer-to-peer support to fellow campaign participants on their unique areas of expertise
  • 12 organizations successfully completed participation in the Innovator Track Cardiovascular Disease (CVD) Cohort and collectively improved in six CVD-related measures
  • 30 partners, experts, and T2G participants presented on 16 T2G and Innovator Track webinars
  • 30 diabetes teams hosted a T2G Talk & Taste for this year’s National Day of Action, engaging more than 300 healthcare professionals in conversations on diabetes improvement

Plank Mentor of the Month: Frank Colangelo, M.D., M.S.-HQS, FACP

Campaign Plank: Conduct Practice-Based Screening

Dr. Colangelo practices as an internist and has served as chief quality officer of Premier Medical Associates since 2008. He has led numerous successful population health and quality improvement efforts for the practice. He earned a Masters in Healthcare Quality and Safety from Thomas Jefferson University’s College of Population Health, and is currently an instructor for the college. Contact Dr. Colangelo today!
