Campaign Toolkit & New Supplement

The Together 2 Goal® Campaign Toolkit guides participating AMGA members in implementing the campaign planks, or evidence-based care processes, to more effectively manage patients with Type 2 diabetes. This free resource features proven, practical tools from fellow AMGA members to help campaign participants address many of the common challenges associated with diabetes improvement.

With the launch of the 2019 extension of the Together 2 Goal® Campaign, the T2G Team developed an additional Toolkit Supplement. Outfitted with new tools used by participating members, the Toolkit Supplement is intended to provide additional support as groups work to refine and hardwire their organization’s improvement processes over the course of the campaign extension.
