Innovator Track Eye Care Cohort Case Studies [+]
From July 2018 to September 2019, the Innovator Track Eye Care Cohort (Eye Care Cohort)
developed and implemented care processes that sought to improve documented screening rates for diabetic retinopathy for patients
with diabetes. The case studies below detail the interventions, outcomes, and lessons learned from each medical group and health
system that participated in the Eye Care Cohort.
Innovator Track CVD Cohort Case Studies [+]
From May 2018 to May 2019, the Together 2 Goal® Innovator Track
Cardiovascular Disease (CVD) Cohort explored how providers can
better identify and manage CVD risk among their patients with Type 2
diabetes. In these case studies, medical groups and health systems
provide insight into their interventions, outcomes, and results.
