Our 11 campaign planks (evidence-based care processes) guide campaign participants in improving care for people with Type 2 diabetes. Organized by three domains, these planks are based on insights gleaned from our Best Practices Learning Collaborative on diabetes and in consultation with our National Advisory Committee and Scientific Advisory Committee.

Participating medical groups and health systems commit to implementing at least one campaign plank, although many are implementing multiple planks and some have adopted all 11.

Download a two-page overview of the campaign planks or view a detailed description of each campaign plank with accompanying resources. Further information on each campaign plank is also available via recorded monthly campaign webinars, and you can learn about success stories on specific campaign planks from our Goal-Getters.
